Patient Forms
Upon entering our office, our receptionist will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.
We realize that this is your first visit to our office and our past experience has shown us that new patients have many unanswered questions on their minds. Our staff will attempt to do everything possible to ensure that you feel at ease and to answer any questions that you may have.
To ensure that your first visit with us is a pleasant one, here are the procedures that you may expect during this visit.
Consultation
You will have a consultation with Dr. Bobo to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine if yours is a chiropractic condition. You will have time to discuss your health concerns at this time.
Examination
After your consultation, Dr. Bobo will perform a complete chiropractic examination. Other standard neurological, orthopedic, and physical tests will be performed to determine the cause(s) of your problem. However, nothing will be done in our office without your consent.
Future Visits
This will conclude your initial appointment. Plan to spend approximately 30 minutes on your next visit to receive Dr. Bobo's findings and a chiropractic adjustment, if needed. Details regarding treatment, expectations, prognosis, health coverage, financial issues will be discussed in detail before your case is accepted for treatment.
